This Excel-based tool is designed to support patient navigators, providers, and other staff who are responsible for coordinating patient access to, and the procurement/storage of long-acting injections for HIV treatment or prevention (e.g., lenacapavir, cabotegravir). Use this tracker to: document pharmacy and coverage information, track prior authorization and prescription status, and ensure medication is on hand for upcoming patient injection visits.
