  • Abounding Prosperity

    Project Champion: Melissa Curry

    Founded in November 2005, Abounding Prosperity, Inc. addresses the social and health disparities impacting Black men and their families in Dallas County. The organization provides HIV and STI testing, treatment, prevention, and care services; vaccine access and equity initiatives; emergency rental and utility assistance; a transgender ID program; and other health, education, and support services for high-risk populations across southern Dallas County. Our vision is to empower clients to achieve self-sufficiency through skills development, increased knowledge, and access to vital community resources.

  • Baltimore City Health Department

    Project Champion: Lola Akola

    The Baltimore City Health Department is the oldest continuously-operating health department in the United States, formed in 1793 when the governor appointed the city's first health officers in response to a yellow fever outbreak in the Fells Point neighborhood. During the more than 220 years since then, we’ve been working to improve the health and well-being of Baltimore residents. We strive to make Baltimore a city where all residents realize their full health potential.

  • Coastal Family Health Center

    Project Champion: Will Holt

    Coastal Family Health Center (CFHC), the fourth-largest community health center in Mississippi, provides care through 12 clinics, 19 school-based sites, a mobile unit, 4 pharmacies, and 2 administrative offices across six Gulf Coast counties. For over 45 years, CFHC has offered comprehensive medical services and, for 24 years, HIV care under the federally funded Ryan White Program. Accredited by the Joint Commission and recognized as a Patient-Centered Medical Home by the National Committee for Quality Assurance, CFHC remains dedicated to accessible, high-quality healthcare.

  • Florida Department of Health Sunshine Care Center

    The Department of Health in Orange County (DOH-Orange) has been providing HIV patient care services since 1989 through the Sunshine Care Center (SCC). DOH-Orange leads the way in providing healthcare and assuring that the health needs of our community are met. DOH-Orange, through the SCC, remains dedicated to providing HIV primary and supportive care in an outpatient setting. SCC is the primary source of care for uninsured and under-insured women, men, infants, youth, minority populations, and families infected with and affected by HIV in Central Florida.

  • Mount Sinai Institute for Advanced Medicine Jack Martin Fund Center

    Project Champion: Levi Solimine

    The Institute for Advanced Medicine (IAM) was developed in 2013 when Mount Sinai Hospital merged with Continuum Health Partners to form the Mount Sinai Health System, with the goal to improve quality outcomes across the Health System’s clinics serving PWH. IAM includes five clinics within four hospitals, all of which were named “Designated AIDS Center” by the Department of Health AIDS Institute in the late 1980s. Co-located services include HIV counseling and testing, PrEP and PEP, STI and HCV screening and treatment, gynecology, sub-specialty care, mental health services, medical case management, health education and risk reduction, pharmacy services, and trauma and domestic violence screening

  • Positive Impact Health Centers

    Project Champion: Shin Ly

    Positive Impact Health Centers (PIHC) is a private non-profit organization serving the greater Atlanta, GA area. For over 30 years, PIHC has provided services to people living with and at risk of HIV with Centers located in Decatur, Duluth, and Marietta. PIHC is accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF) for outpatient ambulatory care, HIV prevention, case management, behavioral health, and substance abuse services and is also certified as a Patient Care Medical Home (PCMH).

  • San Antonio AIDS Foundation

    Project Champion: Fabriel Torres

    SAAF was the first community-based organization created in response to the epidemic of HIV/AIDS in San Antonio, Texas. Founded in 1986, SAAF provided residential nursing and hospice services for 30 years and continues to provide support for over 2000 individuals living with HIV/AIDS. In 2017, SAAF closed its residential nursing facility and transitioned to a community-based outpatient HIV clinic. With a staff of nearly 60 individuals, SAAF provides daily meals, case management, mental health services, oral health care, testing (HIV, Syphilis, Gonorrhea, and Chlamydia), outreach, and education. Wrap around services also include rental assistance, transitional housing, emergency financial assistance, medical transportation, and substance use counseling

  • Sinai Infectious Disease Center

    Sinai Infectious Disease Center

    Project Champion: Bijou Hunt

    The Sinai Infectious Disease Center (SIDC) has been providing HIV care on the west side of Chicago for over 20 years. The communities we serve are predominantly African American and Latinx and include large immigrant and mono-lingual Spanish-speaking populations. We are a Chicago Department of Public Health designated Patient Centered Health Home, primarily funded by Ryan White. Our services include medical care, as well as wrap-around social support services delivered by a diverse team that reflects our patient population.
